The lead GP for this is Dr Qureshi and the lead nurse is Julie . This does not mean they are the only people you are able to see but they are responsible for overseeing the system and planning regarding your child’s asthma.
You will be offered a structured review and individual asthma annual care plan.
Everyone is at risk of developing heart disease, stroke, diabetes or kidney disease.
The good news is that these conditions can often be prevented – even if you have a history of them in your family.

Who is eligible ? Both men and woman between 40 and 74 who has not been diagnosed with the conditions mentioned.
What happens at the check?
- The check will take around 20-30 minutes.
- You will be asked simple questions, for example about your families medical history.
- We will record your height, weight, age, sex and ethnicity.
- We will take your blood pressure.
- We will do a simple blood test to check cholesterol etc, this will be a fasting blood test.

If you need a medical examination for such things as life insurance, HGV license, pre-employment check etc these can be arranged by appointment and will be subject to a fee. The fee must be paid prior to an examination. Please see the receptionists for more information.
If you need any forms filling in and signing by the doctor then there will be a charge. Please ask the reception staff for more information.
Payment is required before the completion of the form, please.
If you require any vaccinations relating to foreign travel please collect and complete a travel questionnaire available at reception, the Practice Nurse will then advise on vaccinations you require, once this has been completed a follow up appointment can be arranged with the Practice Nurse for you to receive any vaccinations required.
This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.
It is important to make this initial appointment as early as possible – at least 6 weeks before you travel – as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

We run a child health clinic on a Tuesday on a weekly basis.
Baby 8-week assessments are carried out at this time and also routine vaccinations. Any further vaccinations are by appointment only with the practice nurse.

We also have our First Contact Practitioner Dave Murray who can advise and treat patients with any Musculoskeletal conditions.
The Health Care Assistant (HCA) will complete blood tests, blood pressure monitoring, Glucose Tolerance Tests, New Patient Health Checks, Health Checks and Weight monitoring.
Our Practice nurses are qualified and registered nurses.
They can help with health issues such as family planning, healthy living advice and blood pressure checks.
The practice nurses run clinics for long-term health conditions such as asthma or diabetes, minor ailment clinics and carry out cervical smears.
